Participation Waiver and Agreement

Instructions: 

  • Please fill out one waiver per person.
  • If you've purchased a seat/ticket for another adult, they will need to complete their own online waiver; please share the link to this page prior to the session/event. If they're unable to complete this online, we will have paper waivers available at the Wellness Studio.
  • If you are a parent or guardian and attending an event that allows minors, please fill out a separate waivier for each minor(s).

1. Scope of Services

I understand that The Harnessed Core offers a variety of wellness-based services.

I understand that participation may involve sitting, standing, gentle movement, guided visualization, breath awareness, emotional reflection, or quiet stillness.

2. Educational & Non-Medical Nature of Services

I acknowledge and understand that all services provided by The Harnessed Core are educational and experiential in nature and are not a substitute for medical, psychological, psychiatric, or other licensed healthcare services.

I understand that:

  • No medical diagnoses are made
  • No medical treatment is prescribed
  • No guarantees of outcome are expressed or implied
  • I agree to consult with my licensed healthcare provider regarding any physical or mental health concerns I may have.

3. Voluntary Participation & Personal Responsibility

I affirm that my participation in any service is voluntary. I am responsible for listening to my body, honoring my limits, and choosing my level of participation.

I understand that I may pause, modify, or discontinue participation at any time and that I am encouraged to do so if something does not feel supportive or appropriate for me.

4. Emotional & Physical Responses

I understand that wellness practices may bring up physical sensations, emotions, memories, or insights. These responses are a natural part of mind-body awareness and vary from person to person.

I acknowledge that:

  • Emotional or physical responses are not harmful in themselves
  • I am responsible for seeking additional support if needed
  • The practitioner is not responsible for how I interpret or act upon insights gained during or after sessions

5. Assumption of Risk

I knowingly and voluntarily assume all risks associated with participation in services provided by The Harnessed Core, whether known or unknown, including but not limited to physical discomfort, emotional responses, or environmental factors related to the space or setting.

6. Release of Liability

To the fullest extent permitted by law, I hereby release, waive, and discharge The Harnessed Core, its owner, practitioners, contractors, and representatives from any and all claims, demands, causes of action, or liability arising out of or related to my participation in services, whether occurring in-studio, off-site, or on private property.

7. Off-Site & Third-Party Locations

I understand that some services may take place off-site or on private property not owned or controlled by The Harnessed Core.

I agree that:

  • I am responsible for my own safety and belongings
  • The Harnessed Core is not responsible for conditions of third-party locations
  • I assume all risks associated with participation at off-site locations

8. No Guarantees

I understand that wellness experiences are individual and subjective, and no specific outcomes are promised or guaranteed.

9. Confidentiality

Personal information shared during one-on-one sessions is treated with care and discretion. However, confidentiality may be limited in group settings, and I understand that The Harnessed Core cannot guarantee confidentiality among group participants.

10. Audio & Video Recording Policy

To protect the privacy, safety, and experience of all participants, audio recording, video recording, photography, livestreaming, or any form of digital capture is not permitted during any services offered by The Harnessed Core.

I agree not to record or transmit any portion of a session using phones, smart devices, cameras, or recording equipment without explicit written permission from The Harnessed Core.

I understand that this policy exists to:

  • Maintain a respectful and distraction-free environment
  • Protect participant privacy and confidentiality
  • Support the integrity of the experience for all involved
  • Failure to comply with this policy may result in removal from the session without refund.

Minor Participant Consent

This section applies only if the participant is under the age of 18.

I am the parent or legal guardian of the minor participant. I give my informed consent for the minor to participate in services offered by The Harnessed Core, including group meditation gatherings, educational classes, workshops, and one-on-one wellness sessions, whether in-studio or at off-site locations.

I understand that these services are educational and experiential in nature and are not medical, psychological, or therapeutic treatment.

On behalf of myself and the minor, I voluntarily assume all risks associated with participation and, to the fullest extent permitted by law, release and waive liability against The Harnessed Core, its owner, practitioners, contractors, and representatives for claims arising from ordinary negligence. This waiver does not apply to gross negligence or willful misconduct.

I affirm that the minor is physically and emotionally able to participate and agree that it is my responsibility to consult a licensed healthcare provider if I have any concerns.

I understand that confidentiality cannot be guaranteed in group settings and agree that the no audio or video recording policy applies to minors.
